Originally Chartered:  Oct 14, 1841 as Coleman Lodge #40

Name Change:  Oct 15, 1855 to Mount Moriah Lodge #40

Mizpah Lodge #639 Chartered Sept 29, 1915

Merged as Mizpah Mount Moriah #40 on Oct 17, 1995
